Friends,
I intend to work in the field of Clin. PK/TDM of anticonvulsants for
my Ph.D. in BioMed. Engg. at India's premier technological institute.
My background is Physical pharmacy and I shall be collaborating with
a large public hospital which will provide immense raw data related
to patient history, diagnosis, treatment suggested and TDM results.I
propose to study the effect of some of the following intra-patient
and inter-patient variables -age, weight, body surface area,
menstruation, nutritional status (anemia), hepatic and renal
impairment, type of seizure, comedication, use of herbal/ Ayurvedic
remedies, caffeine, tobacco on the PK parameters so as to
individualize the dosage regimen. Data is available fot CBZ,
Phenytoin, Sod. Valproate, and Phenobarbitone. I am also interested
in carrying out Pop. PK studies using this data.
Clinical Pharmacy is still in its infancy and computer applications
in PK/TDM is untried in India.Softwares like NON-MEM are not
available. I have no backgrond in designing / using any PK
softwares.No expert help is avilable locally.
However, I feel strongly committed to this field due to its utility
and relevance to patient care, specially in the context of India.
I need your suggestions/ help in the following-
1) My Ph.D. project will be cleared only if there is an element of
innovation / valuable addition to existing academic knowledge
internationally.In the light of the immense work done in this field
worldwide in the last two decades,can my project claim any of it? In
which areas?
2)Can I obtain a standard software free (for academic use only)from any source?
3)Where can I get training in the use of software like NON-MEM and
an orientation to Pop. PK?
My project, (if it materializes) is likely to open the field of
computer applications in TDM/PK in India. I bank on your feedback for
the same.

I think that exciting work is done in Bayesian PK/PD modelling. See referenc=

3rd ed. Applied Therapeutics, Inc., Vancouver, WA, 1992.
In the above chapter on pages 30-18 the authors discuss a method to
determine a patient specific relationship between plasma heparin activity
and APTT, on the next page there is a method by Chiou (eq. 30-3) for
calculating heparin clearance. I would like to know if anyone is using this
method in actual clinical practice and I am very interested in any comments,
concerns, and experience anyone would be willing to share regarding this
PK/PD method of dosage adjustment of heparin.
