I am looking for reference(s) on Vancomycin trough level ranging from
5-15ug/ml or 5-25ug/ml.
For MRSA patients, do you use a different trough goal?

Ikezuagu,
We look for Vanco Cpmin levels at least 4 times the MIC. We also monitor
MIC trends for vanco target organisms. Most organisms here are still
with MIC's of 0.5-1 although there are enough 2's to cause concern.
Empirically, we try to target vanco troughs at 10mcg/ml. This covers
most of what we see and can usually be modeled with standard dosing
regimens.
