Good morning,
When you have a graph with conc.-time at two different times, one on day
1 of treatment and the other on week 13 of treatment, and you can see on
day 1 a "normal" curve but on week 13 it appears two peaks.
It may be due?

Dear Nuria,
The second peak after 13 weeks treatment may be due to the appearance
of a metabolite. It might be due either to metabolite accumulation or
Cyp induction. Did checked CYP induction data you might have generated?

I wonder if this couldn't be a result of enterohepatic circulation as
this process became more evident after multiple-dose regimen.
Do you know, Nuria, if the drug is secreted in bile?

How the second peak can be due to CYP induction?

Hi Nuria, was the same lot#/batch # of API used to dose on both days
and what was the formulation type, e.g. solution, capsules, tablets,
suspension? Also, when you say two peaks, I assume that you are
referring to the parent and not a peak of another chemical entity.
What was the design of the study (composite/serial) and how large is
the peak. Also does this peak appear early on in the profile (how long
are you sampling for?)?
Answering the above questions could probably help the participants of
the forum provide the correct feedback.
A second peak on day 13 and not one day 1 could also because of
formulation issues, e.g. if the formulation on day 1 (tablet, capsules
or suspension) has a larger particle size compared to that
administered on day 1.
