Please excuse the length of this posting. Its important!!
We, in our pharmacy department, have been challenged to reorganize
ourselves using the folling criteria: a) reassuming the duties of medication
order entry, b) increase our role as providers of education to patients,
nurses and doctors, and c) become involved in the direct care of patients up
to and not excluding the administration of medication, (giving injection is
not in the plan, but nothing is off limits.) Currently most (90%)
pharmacist do very little education, and God forbid except for changing PCA
bags and parameters, rarely enter patients rooms.
We are a private, 700 bed, tertiary care, regional medical center. We
have no affliation with any pharmacy school and only a passing association
with a medical school (a couple of residents each month, no fellows or
students). Currently, Clinical Data Tech (CDTs) enter physician orders.
This will end abruptly (high error rate, high turn over, loss of forest due
to paper use.) We have a small clinical staff, 4 FTEs and me.
Whatever plan we come up with, it cannot add a bunch of FTEs.
Our new administrator is very supportive of pharmacy and especially
clinical pharmacy. She recognizes our strengths and our knowledge base.
She is willing to give us a shot at doing this ourselves. If we do not do
the job, someone will do it for us.
I have sent out feelers on reorganization before. This time it is a real
time happening. I was unfortunate enough to be named to the task force to
develope this plan. That happens when you ask the wrong questions and are
committed to continuing employment.
