I was wondering if anyone knew of an "accepted" therapeutic range of
thalidomide for bone marrow transplant recipients (and any references
pertaining to this range)?

As such, I doubt if there is an "accepted" therapeutic range. A couple
of articles have had successes in this vein:
[1] Forsyth, CJ et al., Thalidomide responsive chronic pulmonary GVHD.

[2] Cole, CH et al., Thalidomide in the management of chronic GVHD in
children following bone marrow transplantation. Bone Marrow
Transplantation. 14(6), 937-942, 1994.
You could also call up the BMT unit at the BC Cancer Agency and/or the BC
Children's Hospital to see what range they are using.
